WELLNESS INCENTIVES
What are Wellness Incentives?
An annual reimbursement for covered members who complete one of the eligible screening procedures on your accident, critical illness and cancer plans.

Plan Rules

If you or a covered dependent get one of the eligible screenings, you can file a wellness claim
-
Once approved, you will receive a check for the wellness benefit amount
-
The wellness benefit can be submitted annually as long as your critical illness, accident, hospital indemnity and cancer plans are in force

How to file your Wellness Benefit Claim:

Provide details including: healthcare provider’s name, address & phone number; screening test, date and address where completed. Multiple claims for spouse or dependent children can be included all in one call if currently one if they are included on your plan.
