What are Wellness Incentives? 

An annual reimbursement for covered members who complete one of the eligible screening procedures on your accident, critical illness and cancer plans.

Plan Rules

  • If you or a covered dependent get one of the eligible screenings, you can file a wellness claim

  • Once approved, you will receive a check for the wellness benefit amount

  • The wellness benefit can be submitted annually as long as your critical illness, cancer and accident plans are in force

Wellness Tests:

• Blood test for triglycerides

• Blood test to determine total cholesterol

• Bone marrow aspiration or biopsy

• Breast MRI

• Breast ultrasound

• Breast sonogram

• CA 15-3 (blood test for breast cancer)

• CA 125 (blood test for ovarian cancer)

• CEA (blood test for colon cancer)

• Carotid Doppler

• Chest x-ray

• Clinical testicular exam

• Colonoscopy

• Doppler screening for cancer

• Doppler screening for peripheral vascular disease

• Echocardiogram

• Electrocardiogram

• Endoscopy

• Fasting blood glucose test

• Fasting plasma glucose (FPG)

• Hemoglobin A1C (HbA1c)

• Flexible sigmoidoscopy

• Hemocult stool analysis

• Oral cancer screening

• Physical Exam

• PSA (blood test for prostate cancer)

• Serum cholesterol test to determine HDL and LDL levels

• Serum protein electrophoresis (blood test for myeloma)

• Skin cancer biopsy and screening

• Stress test on a bicycle or treadmill

• Thermography

• Two hour post-load plasma glucose

• Ultrasounds for cancer detection

• Ultrasound screening of the abdominal aorta for abdominal aortic aneurysms

• Virtual Colonoscopy

How to file your Wellness Benefit Claim:

  1. Call MetLife at 1-800-438-6388

  2. Provide details including: healthcare provider’s name, address & phone number; screening test, date and address where completed. Multiple claims for spouse or dependent children can be included all in one call if currently one if they are included on your plan.

OR

  1. Complete Critical Illness or Accident Wellness Claim forms

  2. Mail, Fax or scan & email to MetLife (contact info included on form)

Don’t forget your $50 wellness benefit!  If you are currently covered under the Allstate Cancer Policy, you also receive the “Wellness Benefit” as part of your policy.​  The Wellness Benefit Amount is $50.00 per calendar year for each insured under the Cancer policy.

What qualifies as wellness:

• Biopsy for skin cancer

• Blood tests for triglycerides 

• CA15-3 (breast cancer)

• CA125 (ovarian cancer)

• CEA (colon cancer)

• PSA (prostate cancer)

• Bone Marrow Testing • Chest X-ray

• Colonoscopy

• Doppler screening for carotids or peripheral vascular disease

• Echocardiogram

• EKG

• Flexible sigmoidoscopy

• Hemoccult stool analysis

• HPV (Human Papillomavirus) Vaccination

• Lipid panel (total cholesterol count)

• Mammography, including Breast Ultrasound

• Pap Smear, including ThinPrep Pap Test Serum Protein Electrophoresis (test for myeloma)

• Stress test on bike or treadmill

• Thermography

• Ultrasound screening for abdominal aortic aneurysms
